Tolcapone

Many people taking levodopa for Parkinson's have problems with the effects of the levodopa wearing off between scheduled doses, causing symptoms to return or worsen.
This effect allows the levodopa to last longer in the system so that it doesn't wear off before the next dose.
Tolcapone belongs to a class of drugs known as COMT inhibitors.
Tolcapone blocks a certain natural substance (COMT enzyme) that breaks down levodopa in the body.
Tolcapone is prescribed in combination with levodopa-carbidopa therapy to treat Parkinson's disease.
Tolcapone is prescribed with other medications (levodopa/carbidopa) to treat Parkinson's disease.

Side Effects

Tolcapone side effects that you should report to your health care professional or doctor as soon as possible:
- Abdominal pain;
- Chest pain;
- Confusion;
- constipation;
- dark urine;
- Diarrhea;
- Drowsiness;
- Dry mouth;
- Easy bleeding/bruising;
- Fast/irregular heartbeat;
- feeling faint or lightheaded;
- Fever;
- Gas;
- general ill feeling or flu-like symptoms;
- hallucinations;
- Headache;
- Increased gambling;
- Increased number of dreams;
- increased sweating;
- involuntary muscle movements;
- Itching/swelling (especially of the face/tongue/throat);
- light-colored stools;
- loss of appetite, nausea;
- Mental/mood changes;
- muscle cramping;
- Muscle stiffness;
- Muscle weakness;
- Nausea/vomiting;
- Pain with breathing;
- Rash;
- right upper belly pain;
- severe diarrhea;
- Severe dizziness;
- Sexual urges;
- Sore/painful muscles;
- stomach upset;
- Trouble breathing;
- trouble sleeping;
- Unusual urges;
- unusually weak or tired;
- Unwanted/uncontrolled movements;
- yellowing of eyes, skin;

Contraindications

Tolcapone is contraindicated if you have any of the following conditions:
- an unusual or allergic reaction to tolcapone, other medicines, foods, dyes, or preservatives;
- breast-feeding;
- kidney disease;
- liver disease;
- pregnant or trying to get pregnant;
- severe movement disorder;
- if you are allergic to tolcapone;
- if you have alcoholism;
- if you have any other allergies;
- if you have fever and confusion caused by any drug;
- if you have history of a certain muscle problem (rhabdomyolysis);
- if you have liver problems;
- if you have low blood pressure;
- if you have parkinson's with severe movement problems;

Interactions

Do not take Tolcapone with any of the following drugs:
- Alcohol;
- Alprazolam;
- Antihistamines;
- Apomorphine;
- Certain MAO inhibitors;
- Cetirizine;
- Codeine;
- Diazepam;
- Diphenhydramine;
- Dobutamine;
- Drugs for sleep or anxiety;
- Furazolidone;
- Isocarboxazid;
- isoniazid;
- isoproterenol;
- Linezolid;
- medicines called mao inhibitors like nardil, parnate, marplan, eldepryl;
- Methyldopa;
- Moclobemide;
- Muscle relaxants;
- Narcotic pain relievers;
- Phenelzine;
- Procarbazine;
- Rasagiline;
- Selegiline;
- Tranylcypromine;
- warfarin;
- Zolpidem;
